- Ensure needs are being met including but not limited to their safety and well-being.

- Drive people to and from program in AM and/or PM.

18 hrs | 3-days a Week | 8:30A – 3:30P

$32/hr
Job Description:
Including, but not limited to:
- Identify, develop and coordinate sites within community for people consistent with their needs, desires and valued outcomes.
- Conduct psychological assessments, screenings and evaluations.
- Create, train and assist with implementation of Behavior Modification Plans and/or Strategies and revise as needed.
- Monitor Behavior Modification Plans and/or Strategies and revise as needed.
- Conduct individual and/or group therapy sessions.
- Provide crisis intervention as needed.
- Attend interdisciplinary team meetings, annual and semi-annual meetings and other meetings as required to ensure quality services for people attending programs.
