What is Community Habilitation?

Community Habilitation is an individualized service for individuals who live with their families, loved ones, or independently within the community. Community Habilitation offers so many supports; you pick the ones that work for you! 

Supports include but are not limited to: venturing out into the community, working on skill development, assisting with activities of daily living (hands-on), building relationships, training and supports for independence in travel and transportation, embarking on adult education, developing social and leisure skills, exploring self-advocacy and informed choice skills and facilitating appropriate behavior development to help the individual safely access their community.

Where does Community Habilitation take place?

Community Habilitation can take place in the community and services can also be delivered at any non-certified location.

Who is eligible for Community Habilitation?

To be eligible, individuals must have a Care Manager and be Waiver Services enrolled. Your Care Manager will help with enrollment.

People living in certified residences can utilize Community Habilitation instead of certified day programs Monday – Friday during vocational & day program hours. Services, however, cannot be delivered within their certified residence.

How are Community Habilitation Workers chosen?

The Community Habitation staff work hard to find the right Community Habilitation worker for you. They act like a match maker! The individual to receive services and their family members are interviewed to determine what kind of Community Habilitation worker would suit your needs and wants.  After staff are interviewed, the match making begins and introductions are made. If you discover the match was not made in heaven, no problem!  Just call the Coordinator of Community Habilitation to find another Community Habilitation worker.

If you know someone who would like to become a Community Habilitation worker, you can make a referral.  Please note that family, friends or loved ones who reside at the place of residency for hire are not eligible.
